What we need
- An easily understood visual aid that communicates complex information
- A high-resolution version of the Infographic in the Organization’s preferred format
Additional details
We are under a planning grant to develop an implementation plan that identifies individuals with social determinant needs and connects (links) them with community services that are able to meet these needs in a fully transparent and interactive way
What we have in place
- We currently have a number of descriptor and workflow documents, which should make it easy for you to get started. We also have a project manager and content expert, and the ability to provide any other information you need.
How this will help
This project will save us $4,212 , allowing us to support content experts and demonstrate how this project could link with other public health activities, in hopes of securing staffing of the implementation plan.
Social needs are a key barrier to individuals obtaining and or maintaining healthy behaviors and care. Our community has agreed to undertake the exploration of identifying current state and developing a plan to begin to implement. The concepts are difficult to grasp and a simplified graphic would assist in demonstrating the challenges and goals of the planning
